After Hours Emergency: If you are experiencing a life-threatening emergency call 911 immediately for emergency medical services. For urgent healthcare needs after our office is closed, please contact your nearest, contracted Urgent Care Facility or Emergency Room, then follow up in our office with your Family Physician. Contact your health plan for any additional information you may need to obtain emergency health services.
Choosing a family doctor is a great way to maintain exceptional health throughout the course of your life. Family Physicians specialize in treating the whole person, in the context of their lives. In addition to treating illnesses, your Family Physician can partner with you to meet your long-term health goals and needs. Your ongoing relationship with your Family Physician will help you successfully interact with a complicated health care system, and enhance your health and quality of life. Appointments:
If you are unable to keep your appointment, please notify our office 24 hours in advance so another patient may be seen in your place. We do charge a $20 “No Show” fee. Fees and Payments:
We make every effort at Your Family Physician to keep our fees fair and reasonable. Our policy is to collect any co-payments at the time of your visit. For patients without insurance, who pay at the time of service, we offer a 20% discount on the office visit. For your convenience you can now pay your clinic bills online. This service allows you to pay your bills quickly and securely. Insurance Information:
What do I need to do with a referral to see another specialist? Your Family Physician will occasionally recommend that you see another specialist. To determine whether you need a paper referral from us before seeing another specialist, please view the terms of your insurance plan. Often, this information is listed on your insurance card or can be obtained by calling your insurance company. After you schedule your appointment call Debbie at(623) 561- 9113 Extension 205, so we can confirm all your pertinent health information is sent to the referral specialist. Occasionally, the specialist we recommend is no longer accepting your health plan. Contact your insurance company for additional specialists contracted with your plan. Prescription Refills: If you need a prescription refill before a scheduled appointment, please call your pharmacy. Ask them to send us a fax if additional information is needed. If your prescription has no refills available or has expired, call our office to schedule an appointment. A review of your health status and medication refill appointment generally can be scheduled the day you call. Any changes to a prescribed medication will need an appointment prior to changing the medication. If there is an urgent need and you are unable to make an appointment, you can also renew your prescriptions online through our website. Please allow at least 48 hours for your refill to be processed. No refills for controlled substances will be authorized without an appointment. |
